The order never changes, whether you start at a GP's rooms in Sandton, a clinic in Durban or a telehealth consultation from your kitchen table. The setting is flexible; the sequence is law.

Pharmacies cannot skip the first step. South African law does not allow a pharmacist to hand over a Schedule 4 medicine without a doctor's authorisation, however familiar the medicine has become on social media.

The same law protects you. It means a doctor has checked your history, your other medicines and your suitability before tirzepatide enters the picture.

Who qualifies for Mounjaro in South Africa?

Meeting the BMI number does not guarantee a plan. Doctors also weigh up pregnancy plans, thyroid cancer history in the family, past pancreatitis and interactions with current medicines. Equally, a doctor may decline treatment that a website calculator would have waved through. That judgement is the point of the assessment.

What should you have ready for the assessment?

Your weight history, a full list of your medicines, chronic conditions and past surgeries, family history of thyroid cancer, and recent blood results if you have them. The consultation moves faster and lands better decisions when you arrive prepared. Have these to hand:

• Your weight history: roughly what you have weighed over recent years and what you have tried before, from eating plans to other medicines.
• A list of every medicine and supplement you take, with doses.
• Chronic conditions and past surgeries, especially anything involving your pancreas, gallbladder, thyroid or gut.

Delivery is normal and legal when a licensed South African pharmacy does it. Reputable pharmacies ship pens in insulated cold-chain packaging and tell you the window to expect. Unpack and refrigerate the pen as soon as it arrives.

The test of a legitimate service is simple: a doctor assesses you before any medicine is arranged, the doctor is HPCSA-registered, and the dispensing pharmacy is licensed and named. Expect the consultation to run by video call or a structured medical questionnaire with direct contact with the doctor. Answer everything fully, including questions that feel unrelated. An undisclosed condition or medicine can turn a suitable treatment plan into an unsafe one, and the doctor can only work with what you share.

Both groups stock the Mounjaro KwikPen and can dispense it once a doctor has authorised treatment. The same applies to Medirite, courier pharmacies and independent community pharmacies across the country. What no pharmacy, anywhere, can do is sell it over the counter.

Stock varies by branch and by strength, so phone ahead before you travel. If your strength is out of stock, ask the pharmacy about a transfer from another branch, or ask your doctor whether to adjust the plan. Do not improvise doses from a different pen strength on your own.

Yes. How do you spot an illegal Mounjaro seller?

An illegal seller shows the same signs on any platform: no doctor's assessment before the medicine, a product that does not match the registered KwikPen, shipping from overseas, or a price far below what licensed pharmacies charge. Any one of those is reason enough to walk away.

• Medicine offered with no assessment at all, or an online form that approves everyone in minutes.
• Vials, ampoules or powder to mix yourself. South African pharmacies stock only the KwikPen.

Two budgets: the clinical side and the medicine. Consultation and follow-up fees are set by each practice, so it is reasonable to ask what assessment and follow-up will cost before you book. The medicine is a separate market cost paid to the pharmacy, and it rises as the dose steps up.

Will medical aid pay for any of it?

Usually not for the medicine when the reason is weight management. Obesity is not a Prescribed Minimum Benefit condition, so most schemes exclude weight loss medicine from standard cover, although some fund tirzepatide for type 2 diabetes under chronic benefits and day-to-day benefits sometimes cover the consultations.

What should you discuss with the doctor before starting?

Ask about side effects first. Nausea, diarrhoea, constipation and vomiting are the common ones, mostly during dose increases, and they usually settle as the body adjusts. Agree upfront on how to manage them, what eating patterns help, and when to make contact between scheduled reviews rather than pushing through.

Ask what success looks like, too. In trials, average weight loss ranged from 15% to 21% of body weight over 72 weeks depending on dose, alongside diet and activity changes. Individual results vary, and your doctor should frame targets around your starting point, not a headline number.

Cover the exclusions. Mounjaro is not used in pregnancy or while breastfeeding, or with a personal or family history of medullary thyroid cancer or MEN2 syndrome, and a history of pancreatitis needs a careful discussion.
